I, MANANG JOY BIDAY, of legal age, Filipino and resident of 11237 Apple Dr., Phase 1D, Valley View, Executive Village Cainta Rizal, after being duly sworn to in accordance with law, hereby depose and state that:

1.  That I am the sole and only surviving legal heir of the late Angelo R. Castillo who died two years ago at Quezon City, Metro Manila leaving without any will or debt that has not been settled up to the present.

 

2.   That the said deceased at the time of her death left a parcel of residential land situated at Sta. Maria, Isabela which is more particularly described to wit;

“A parcel of agricultural land situated at Sta. Maria, Isabela, designated as Lot No. 726364023-E-1 part containing an area of Three Thousand Five Hundred Fifty (10,500) square meters bounded on the North by road right of way: on the East by Lot No. 24783023-E-4: on the South by Lot No. 43437023-D: and on the West by Lot No. 43074023-E, part covered by TCT No. 789398435416 issued by the register of Deeds, Ilagan City, Province of Isabela, declared in the name of Angelo R. Castillo with an assessed value of P 50,000,000.00”

3.  That pursuant to Sec. 1 Rule 74 of the Revised Rules of Court of the Philippines, I hereby Adjudicate unto myself the above described parcel of land without judicial proceedings as prescribed by the aforementioned Rules of Court.

1.     I am the owner of Professional Regulatory Commission Identification Card (PRC ID) with License No. 0000000 issued by the Board of Psychometrician.

2.     The last time I used the said PRC ID is during my bank transaction with BDO Estancia Pasig Branch on 15 January 2021.

3.     Sometime last week, I noticed that the said ID is no longer in my possession.

4.     I called  Estancia Pasig  to inquire as to whether there is an ID left thereat under my name but they answer was in the negative.

5.     I exerted diligent efforts to find and/or locate the same, but the same was proven futile;

6.     As such, I believe that the PRC ID with License No. 0000000 is now lost and beyond recovery;

7.     I am executing this affidavit to attest the veracity of all the above facts, and to request the issuance of PRC ID with License No. 0000000 anew.

I, MANANG BIDAY B. BIDAY, of legal age, single, Filipino, and currently residing at Bahay Kubo Kahit Munti, Ang halaman doon ay Sari-Sari, Philippines, after being sworn in accordance with law, hereby depose and state that:

 

1.     I am the owner of Sim Card with No. 099500000000.

2.     I used the Sim Card No. 099500000000 to register my old account in Gcash.

3.     Sometime on January 22, 2021, my cellular phone where my Gcash application was installed using the sim card, was lost inside our house.

4.  On the date my phone was stolen, my old Gcash account has a remaining balance of approximately Php8,000.00 plus.

5.     I exerted diligent efforts to find and/or locate the same, including reviewing the CCTV Footage, but the same was proven futile;

6.  As such, I believe that the Sim Card with No. 099500000000 is now lost and beyond recovery;

7.   I am executing this affidavit to attest the veracity of all the above facts, and to request that the remaining balance in my old GCash account in the amount of approximately Php8,000.00 plus using my lost Sim Card with No. 099500000000 be transferred to my new Gcash account.
